Referral Information

Almost all pathology requests are now made using the ICE requesting system.

ICE requesting sheets have four integral labels for specimens and have a manual form on the reverse, which should normally only be used in the event of an ICE system failure.

ICE pouches - there are now five different coloured ICE pouches:

GREEN - for routine clinical chemistry and haematology requests

RED - for urgent clinical chemistry and haematology requests

YELLOW - for INR / warfarin requests

BLUE - for microbiology and immunology requests

WHITE - for histopathology requests

Courier bags - there are three different coloured courier bags in use:

GREEN - for clinical chemistry, haematology (including INRs) and histopathology requests

BLUE - for microbiology and immunology requests

PINK - for cervical cytology requests – for transit on to Newcastle.

All courier bags, including the pink cytology bags must be sealed before they leave the practice. This will facilitate the timely and secure transport of specimens to the correct laboratory and ensure the correct specimens accompany the correct request to the correct location.

Blood transfusion requests should continue to be made using the existing blood transfusion request forms.
