District Nursing and Community Matrons

  • Description

    District nursing services provide holistic assessment and nursing interventions to patients in their own homes, residential care home or clinic environment.

    Community matrons provide clinical assessment and interventions to patients with long term conditions who are at risk of frequent admissions to hospital due to unstable disease and risk of exacerbation.

    There are also health and wellbeing community matrons who care for patients with severe mental illness - for further information please see relevant section.

  • Patient types/symptoms

    Patients registered with a GP in Northumberland or North Tyneside over the age of 18 requiring nursing care or assessment of health needs

  • Location

    Various locations in Northumberland and North Tyneside aligned to GP practices and the team also visit patients' homes, residential care settings and GP surgeries or clinics.

  • Opening Times

    District Nursing- 24 hr service

    8.30-7pm, then 7pm–8.30am and Saturday and Sunday via NDUC

    North Tyneside
    8.30-5pm then 5pm -8.30am and Saturday and Sunday via NDUC

    Community matrons Monday to Friday, 8.30- 5pm
