South East Northumberland Community Palliative Care
Palliative care aims to improve the quality of life of patients and their families facing problems associated with any life-threatening illness. We work alongside primary care teams and other specialists involved in the patient's care, so that people can receive palliative care at the same time as receiving active treatment for their condition. The service aims to actively manage problems and help people live as well as possible. The service also supports families in dealing with the patient's illness and in their own bereavement. All patients are under the care of the full consultant-led multidisciplinary team, and their case will be discussed regularly at our MDT meeting.
We accept any patient (and their family) who has a life-limiting illness and who has complex physical, social, psychological and spiritual needs.

Monday - Friday 8.30am - 5.00pm
Outside of these hours, professionals can access the Palliative Care Advice Line on 0191 273 3435. This line is covered by nursing and consultant staff.
